Representative Application Packet

Application information, including the password for access to the application form, was mailed to Carrousel Coordinators on Friday, September 9. Please contact the Carrousel office if you have not received the information by the middle of the following week.

Instructions for completing the application are in the packet. All applications must be mailed to the Carrousel office. No email submissions will be accepted.

Applications must be received at the Carrousel office no later than 4pm on Friday, October 7. NO EXCEPTIONS.
